I cannot answer any of your questions directly, but I can come
close or raise new questions for you.  :-)

One theory of MS is that it is a survival trait just as sickle
cell anemia is a survival trait.  The most common disease of
young people in Africa was Malaria.  A person with Sickle Cell
Anemia cannot contract Malaria.  MS is said to be an over active
immune system that protects us from the various plagues that went
through Europe.  Mother Nature does not care about individuals,
she only cares about the species.  I do not believe this theory
of MS, but you did ask.

I wish I could remember the details of an article I read.  My
(perhaps faulty) recollection is that our old friend Vitamin D is
essential for shutting down the attack of the T-cells against
foreign invaders.  I emphasize that my memory might be faulty.

Just more for you to think about.
