Torn Foreskin, or Torn Head Skin?

"my son who is now 20months old was 8 weeks premie.  When I bathed him
last night I noticed he has a what looks like a tear 'at' the head of
his ***** just where the head of his ***** and the foreskin join is
this normal ????I dont remember this happening to my older son he
isn't cir***sized..."

The question is impossible to answer without more information.  You
are not telling us if the younger son is also uncir***cised, and if so
whether his foreskin is retracted or not.
Let's go with the most likely, that he is uncut and his foreskin is
not yet
retractable or is not retracted.  In that case, you are not able to
see where the foreskin attaches to the *****, behind the ***** head,
because that part is covered from view.  So let's assume you mean the
tip of the foreskin which is still joined to or near the tip of the
head, due to childhood adhesions.  If so, these adhesions may be
separating as they normally will over some period of time, but often
not this early.  A tear in the foreskin in this area can also be the
result of phimosis, which is due to the opening at the tip being too
small to pass the head through it (in the event there are no adhesions
as mentioned earlier).  You or the doctor will need to determine which
of these two common but different foreskin problems is the reason,
before you can treat it.  Meanwhile, Neos****in.
